CUA Form I-9 Checklist

 

Section 2

  • Complete Section 2 by reviewing the document(s) presented, then sign and date the form.

  • Do not backdate forms.  Although the law requires that the I-9 form be completed within three business days of the date the individual is hired, the penalty for false statements is a fine or imprisonment or both.

  • If the individual is hired for three days or less, complete the form at the time of hire.

  • For F-1 students working off campus or engaged in curricular practical training, an I-20 student identification, endorsed with authorization by the Designated School Official, is required and the information should be recorded in the left-hand margin of Section 2.

  • For J-1 visitors, a DS-2019 (formerly IAP-66) must be presented along with Form I-94, and the information should be recorded in the left-hand margin of Section 2.  J-1 students working outside their program must present a letter from their school’s Responsible Officer as well.
