General Assumption of Risk and Waiver: To be used by faculty conducting off campus field trips. Waivers must be obtained for voluntary trips, mandatory high-risk trips, and if a student will participate in voluntary activities while on a mandatory trip.
Volunteer Agreement: To be used by faculty that use volunteers from time to time without compensation but, who are not CUA students or employees.
