CUA Form I-9 Checklist

 

Section 1

  • When applicable, ensure that the new employee’s maiden name is provided.  When it is not applicable, write "N/A" or draw a line through the box.

  • Ensure that the new employee’s address, date of birth, and social security number are included.

  • If the new employee’s application for a social security number is pending, write "pending."  The new employee may call 1-800-772-1213 three days after his or her application for a social security number has been filed to obtain the social security number over the phone after providing verification as to identify.

  • Be aware that U.S. citizenship belongs not only to persons born in the U.S., but also to all individuals born to a U.S. citizen, and to those born in Puerto Rico, Guam, the Virgin Islands, the Commonwealth of Northern Mariana Islands, American Samoa, and Swains Island.

  • If the box "a lawful permanent resident" is checked, ensure that the alien number is entered in the space provided if it appears on a document that is presented, or in Section 2 or Section 3 of the form.  You may not ask the employee to present a document to verify the number if that particular document is not presented.

  • If the box "an alien authorized to work until" is checked, ensure that any alien or admission number is entered in the space provided if it appears on a document that is presented, or in Section 2 or Section 3 of the form.  You may not ask the employee to present a document to verify the number if that particular document is not presented.

  • Ensure the new employee dates Section 1 of the Form I-9.

  • Ensure that the preparer/translator provides his or her name, address, signature, and date, if any person other than the employee prepares Section 1.  That person may be another employee, if that employee acts as a preparer/translator.

  • A preparer/translator may only be used if the new employee is unable to complete Section 1 on his or her own.

  • Ensure that the new employee signs Section 1 even if the information is entered by a preparer/translator.

  • Only the new employee may legally make changes to Section 1 of the form.

  • If the employer types Section 1 of the form for the new employee, then the person doing the typing must complete the information in the preparer/translator box.
